ABSTRACT

Objective To investigate the effect of community diabetic prevented and treated by integrated traditional Chinese and western medicine based on TCM constitution identification. Methods There were 700 patients with diabetes aged 45 to 65 by cluster sampling in community, divided into two groups:experimental group and observation group. The observation group was treated with standardized management and treatment of diabetes. The experimental group was in addition to treated with standardized management and treatment of diabetes, but also given targeted Chinese medicine health guidance and intervention based on TCM Constitution identification. The research last for one year. To observe the fasting blood glucose, 2-hour postprandial blood glucose, glycosylated hemoglobin and diabetic symptoms scale of the two groups before and after intervention. Results The fasting blood glucose and 2-hour postprandial blood glucose had significant difference before and after the intervention (P<0.01), between the experimental and observation group (P=0.001, 0.014). It had significant differences between the two groups in postprandial 2 h blood glucose by analysed of interaction comparison (P=0.002). The scores of diabetic symptoms had significant difference before and after the intervention in different groups (P<0.01). And it also had significant differences in analysis of interaction comparison (P<0.01). Before and after the intervention of diabetes symptoms curative effect index difference was statistically significant (mann whitney U=-2.302, P=0.021). Conclusions Community diabetic prevented and treated by integrated traditional Chinese and western medicine based on TCM constitution identification plays a certain role in the control of blood sugar. It can effectively improve the patient's disease symptoms. While the mechanism still need further study.

ABSTRACT

Objective To investigate the distribution about TCM constitution types and quality of life of the elderly patients with high incidence of tumor in Changning district of Shanghai, and analyze the correlation of them, in order to provide reference for follow-up management and rehabilitation of the patients.Methods The data about lung cancer, gastric cancer, and colorectal cancer was collected from Changning District of Shanghai from Jan. 2013 to Oct. 2015. The classification of TCM constitution was for the TCM constitution identified, and the Karnofsky Performance Status (KPS) was used for the assessment of life quality. Data was analyzed by SPSS 23.0. Results This study had collected 673 patients with tumor, and the average age was 75.82 ± 6.73. The colorectal cancer accounted 56.6%, the lung cancer 26.9%, and the gastric cancer 16.5%. The 88.3% of the patients were Qi-deficiency, and 41.4% belonged to compound constitution. The 53.3% of the patients had 80 scores on the first follow-up KPS value and 54.6% of them had the same scores on the last follow-up KPS value. There was a significant difference in KPS scores between the two follow-ups (rs=0.112, P=0.002). There was a negative correlation between the Yang-deficiency constitution and the Blood-stasis constitution of gastric patients with its first KPS value. Conclusions The TCM constitution types of the elderly patients with high incidence of tumor was Qi-deficiency type, which its KPS value of the two follow-ups were all high. There was a certain correlation between TCM constitution and KPS. Therefore, during the follow-up management of tumor patients, according to the types of TCM constitution, the Chinese medicine rehabilitation program could be set in order to improve their quality of life.

ABSTRACT

Objective: To observe the clinical efficacy of acupuncture plus bloodletting therapy in treating insomnia in people with blood stasis constitution. Methods: Seventy-two patients were randomized into a treatment group and a control group by using the random number table, 36 cases each. The treatment group received acupuncture plus bloodletting therapy, while the control group was intervened by oral administration of estazolam. One-week treatment was taken as a course, for 3 successive courses in total. A follow-up study was conducted 3 months later. The Pittsburgh sleep quality index (PSQI) was evaluated before and after the intervention, as well as in the follow-up. The clinical efficacies were also compared. Results: The total effective rate was 88.9% in the treatment group versus 83.3% in the control group, and the between-group difference was statistically significant (P<0.05). After the intervention, the PSQI scores dropped significantly in both groups (both P<0.05); the between-group difference in PSQI score was statistically significant (P<0.05). The follow-up study showed that the between-group difference in the global score of PSQI was statistically significant (P<0.05). Conclusion: Acupuncture plus bloodletting therapy can produce a more significant efficacy than oral administration of estazolam in treating insomnia in people with blood stasis constitution.

ABSTRACT

Objective To investigate the distribution of TCM Constitution in hypertensive patients with diabetes mellitus by means of cross-sectional study.Methods A total of 150 hypertension patients from Shaanxi university of traditional Chinese medicine and Xianyang central hospital were selected from year 2014-2015,and were divided into two groups, including 62 patients complicated with diabetes, as the observation group; 88 patients without diabetes, as the control group. The distribution of TCM Constitution in the two groups was compared. Results The proportion of Yi n deficiency constitution, phlegm dampness constitution in the observation group was relatively high,but the proportion of Q i deficiency and Yi n deficiency in the control group was relatively high. Compared with the female patients,the proportion of Yi n deficiency constitution,phlegm dampness constitution and blood stasis constitution of the male patients was higher,while the Qi depression constitution and the proportion of the specific physical constitution were relatively low, and the difference was statistically significant (χ2value were 4.408, 3.723, 3.723, 3.964, 3.893, P value were 0.036, 0.044, 0.044, 0.046, 0.039). The factors like gender, age, BMI, alcohol intake, history of coronary heart disease, diet and taste preference were correlated with the physical types (B value were 1.967, 0.089, 0.017, -0.871, -0.959, -0.014,P<0.01 or P<0.05), and the taste preference and history of coronary heart disease posed a negative correlation with all types of constitution. Conclusions The types of TCM Constitution in hypertensive patients with diabetes are mainly Yi n deficiency and phlegm dampness, and the patients with hypertension are the deficiency of Q i and Yi n deficiency. The gender, age, BMI, alcohol consumption, coronary heart disease, and dietary preferences can influence the physical type of constitution.

In terms of lifestyle, the differences in physical constitution between different tastes were statistically significant (χ2= 480.403,P<0.05); exercise time and sleep (P<0.05), and the difference was significant (P<0.001) the relationship between yin deficiency and exercise time (P<0.001); phlegm-dampness and exercise time and sleep time (P=0.009,P<0.001,P=0.001); qi stagnation and dietary hobby and sleep time (P<0.001); special temperament and diet and sleep time (P values were 0.001, 0.008, respectively).Conclusions The constitution type of TCM in hypertensive patients is related to sex, age, educational level and previous diseases, and is affected by diet, exercise time and sleep habits

ABSTRACT

The traditional Chinese medicine (TCM) constitution theory is an important means to reveal the host′s genetic characteristics from the perspective of TCM. The types of constitution can influence liver pathological changes and the prognosis of disease in patients with chronic hepatitis B virus infection, and to a certain degree, they are associated with the polymorphisms of genes involved in immunoregulation, for example, human leukocyte antigen(HLA) class Ⅱ gene. Yin-deficiency constitution is associated with various adverse clinical outcomes, as well as the genotypes of genes including HLA-DQA1*0501. Preliminary data also show that the chronic hepatitis B (CHB) patients with yin-deficiency constitution have a poor response to interferon therapy. This article reviews the current application of TCM constitution theory in the diagnosis and treatment of CHB, its potential value, and existing problems.

ABSTRACT

Objective To investigate and analyze TCM syndrome characteristics, impact factors and internal relationship among the factors of mental fatigue of university students. A preliminary understanding of mental fatigue situation of university students and prevent diseases was acquired in advance. Methods 100 students were randomly selected in Beijing University of Chinese Medicine, of whom 40 were undergraduate students and 60 were post-graduate students. Mental fatigue questionnaire was used to investigate mental fatigue situation for students, and TCM constitution scale was applied to collect TCM syndromes. A body energy survey was also conducted. Results In undergraduate and post-graduate students, the mental fatigue rate was relatively higher, both of them were over 50%of the total numbers, 57.5%(23/40)、61.7%(37/60) respectively. For the fatigue levels distribution in these two groups, the ratios of slight mental fatigue (undergraduate:52.5%;post-graduate:56.67%) were both higher than those of severe mental fatigue (undergraduate:5%;post-graduate:5%). In the undergraduate group, the top three biased constitutions among mental fatigue students were yang deficiency (7 out of 23:30.43%), qi deficiency (3 out of 23:13.04%) and dampness-heat (3 out of 23:13.04%). In the post-graduate group, the top three biased constitutions were yang deficiency (11 out of 37:29.73%), qi deficiency (6 out of 37: 16.22%) and dampness-heat (5 out of 37: 13.51%). Conclusion It is easier for university students with biased constitution to suffer from mental fatigue, mostly with yang deficiency, qi deficiency and dampness-heat constitution.

ABSTRACT

Objective To evaluate the effect of Eight Section Brocade on the lipid metabolism of people with mild dyslipidemia and phlegm dampness constitutionin communities.MethodsA total of 121 residents from the maple street community health service center and TCM outpatients in theXuhui district of Shanghai. Residents were randomizedly into intervention group with 61 residents and control group with 60 by the random number table method. Residents of control group received health education intervention daily, while residents of intervention group practiced Eight Section Brocade besides the education, All the residents continued their intervention until the 24thweek, and the index of blood lipid were evaluated.ResultsThe levels of serum TC 4.15 (2.06, 2.06) mmol/Lvs. 5.34 (3.18, 3.18) mmol/L,Z=-2.113; TG 1.55 (0.39, 0.39) mmol/Lvs. 1.77(0.38, 0.38) mmol/L,Z=-1.289; LDL-C 2.20 (0.42, 0.42) mmol/Lvs.2.50 (0.87, 0.87) mmol/L,Z=-1.237 of intervention group were significantly lower than those of the control group (P<0.05). The total effective rate of intervention group was significantly higher than the control group (91.8%vs. 76.7%;χ2=4.155,P=0.042). Conclusion The Eight Section Brocade with health education can significantly improve the community residents of lipid metabolism in Dampness-phlegmconstitution.

ABSTRACT

Objective To study the effects of Yizhichan moxibustion on chronic fatigue syndrome (CFS) athletes.Methods 143 patients of CFS were blind randomly devided into3 groups, which were moxibustion of Yizhichan (56 cases) group, general moxibustion group (42 cases), and normal group (45 cases).The normal group was treated by passive massage for relaxation, the moxibustion of Yizhichan group was treated by specific acupoints application of moxibustion of Yizhichan in addition to passive massage;and the general moxibustion groupwas treated by general moxibustion in addition to passive massage.All sample were treated once a day for 6 days in a week.3 weeks was a period of treatment, and the total time of treatment was 15 weeks.Results After treatment, the sleep quality (1.73 ± 0.73, 1.54 ± 0.85 vs.0.83 ± 0.52, F=14.130), time for sleep (1.57 ± 0.60, 1.35 ± 0.60 vs.1.03 ± 0.54, F=5.386), sleep time (1.78 ± 0.71, 1.77 ± 0.61 vs.0.67 ± 0.65, F=19.811), daytime dysfunction (0.78 ± 0.83, 1.64 ± 0.83 vs.1.03 ± 0.83, F=4.798) in the control group, moxibustion of Yizhichan group and a general moxibustion group were statistically significant(P<0.05).The total effective rate of the moxibustion of Yizhichan group was better than the control group and the general moxibustion group (83.8%, 42.2% vs.63.6%;x2=13.973, P=0.001).Conclusion Specific acupoints moxibustion of Yizhichan is a beneficial exploration of moxibustion methods of traditional Chinese medicine, and can improve CFS athletes level of physical health, attention and sleep quality.
